Flu Vaccinations for Children in Reception Class up to and Including Year 5

Have you consented for your child to receive their flu vaccination?

If not please visit www.hdft.nhs.uk/nasalflu-consent no later than 7th October (Date has been extended). You will be asked to enter a school code which is: 31555.

The vaccination is free and given via a quick and simple spray up the nose. This will help to protect your child against flu which can be an unpleasant illness and sometimes cause serious complications. Having it will also help protect more vulnerable family and friends.

If we do not have your consent the Immunisation Team will be unable to give it to your child on 19th October.

If you have any questions or do not have internet access to complete the form please contact the team on: 0300 0030 013
